How to prepare for your first visit?
Whether it’s your first time or a check-up visit, there are a few things they need to keep in mind when they prepare for your visit. If you’re ready to make an appointment please read the information below.
To make your visit more efficient we would like to have as much information about you as possible.
- If you have been following with a rheumatologist in the past, please send us your previous records.
- Please bring records of any previous laboratory and imaging studies done.
- Please bring your medication bottle and a list of previous medications prescribed (even if you are no longer taking them).
We look forward to seeing you in our office.
Our Hours
We are open Monday through Thursday from 8:00 AM to 5:00 PM, Friday from 8:00 AM to 12:00 PM and closed Saturday and Sunday.
English and Spanish
Our staff is bilingual. We want you to feel comfortable communicating with us either in English or Spanish.
Insurance
If you do not see your plan, please contact our office prior to your visit and we can confirm if we are in network for you insurance.
You will need to provide your insurance card(s) and photo identification.
Cancellation/No Show Policy
If you are unable to keep your appointment, please notify us as soon as possible, but at least 24 hours in advance. We appreciate your consideration and cooperation.
Existing patients who do not show or cancel within 24 hours of their scheduled appointment will be charged a $25 no show fee.

Do You Need a Referral?
Some health plans require you to provide us with a referral from your primary care provider. Kindly confirm if your health insurance plan required a referral prior your visit.
