How to prepare for your first visit?

Whether it’s your first time or a check-up visit, there are a few things they need to keep in mind when they prepare for your visit. If you’re ready to make an appointment please read the information below. 

To make your visit more efficient we would like to have as much information about you as possible.

  • If you have been following with a rheumatologist in the past, please send us your previous records. 
  • Please bring records of any previous laboratory and imaging studies done.
  • Please bring your medication bottle and a list of previous medications prescribed (even if you are no longer taking them).

Our Hours

We are open Monday through Thursday from 8:00 AM to 5:00 PM, Friday from 8:00 AM to 12:00 PM and closed Saturday and Sunday. 

English and Spanish

Our staff is bilingual. We want you to feel comfortable communicating with us either in English or Spanish.

Insurance

If you do not see your plan, please contact our office prior to your visit and we can confirm if we are in network for you insurance.

You will need to provide your insurance card(s) and photo identification.

Cancellation/No Show Policy

​​If you are unable to keep your appointment, please notify us as soon as possible, but at least 24 hours in advance. We appreciate your consideration and cooperation.

 

Existing patients who do not show or cancel within 24 hours of their scheduled appointment will be charged a $25 no show fee.

Do You Need a Referral?

Some health plans require you to provide us with a referral from your primary care provider. Kindly confirm if your health insurance plan required a referral prior your visit.

We want to share an important update regarding our insurance participation. As of September 15, 2026, The Institute of Rheumatology & Lifestyle Medicine has transitioned to independent insurance contracts.

First and foremost, we want to sincerely thank you for the trust you have placed in our practice. We understand that insurance changes can be stressful, and we are truly sorry for any inconvenience or uncertainty this transition may cause. Our greatest priority has always been to provide you with the highest quality care, and we know how important it is to maintain continuity with your healthcare providers.

Although we have applied to participate with numerous insurance carriers, insurance companies determine which providers they approve and when those contracts become effective. At this time, the following are the ONLY insurance plans that have approved our practice for participation:

Please know that we are actively working with additional insurance companies every day and remain hopeful that more contracts will be approved. Because these decisions are made entirely by each insurance carrier, we cannot guarantee approval or provide a timeline. However, we felt it was important to communicate this information as early as possible so that you have adequate time to plan for your future healthcare needs.

If your insurance plan is not currently listed, please contact our office. We will gladly review your options with you, including our discounted self-pay rates, for patients who wish to continue their care with our practice.
